Understanding Revival’s Care Model
Revival is a direct-pay practice. Membership and consultation fees are paid directly to the practice and are not billed to insurance.
Many patients continue to use insurance for labs, imaging, prescriptions, and outside referrals. Depending on your plan, certain services may also be eligible for out-of-network reimbursement. Revival partners with Reimbursify to help patients submit eligible claims, and HSA/FSA funds may be used for eligible expenses.
